Vaccine Clinic Information
First and Second Dose

Our practice is pleased to be able to offer Covid-19 vaccination to our patient families. Please read on for detailed information on our clinic dates, scheduling, required documentation, and processes.

 

If it has been 5 months since the second dose in the patient's original Pfizer Covid Vaccine series and you would like to receive a Pfizer BOOSTER vaccine for a patient age 12 and older, you may click below for Healow online scheduling, or you may call our office at 309-662-0504 and select option 3 for scheduling.

 

BOOSTER dose Online Scheduling

 

If you are interested in a Pfizer BOOSTER vaccine for your 5-11 year old immunocompromised child, please call our office at 309-662-0504 and select option 1 to speak with a pediatric nurse.

 

For all other Covid Vaccine appointment requests, please call the office at 309-662-0504 and select option 3 for scheduling.

 

 


Scheduling

In an effort to make the scheduling process easier for both our patient families and our scheduling staff, we will be opening up an online scheduling option through Healow.
 

  • You will schedule an appointment for the first dose in the series.
  • We will automatically schedule the second dose in the series for approximately 3 weeks later at the same appointment time you selected for the first dose.
  • You will receive an appointment confirmation for the second dose.
  • We may not be able to accommodate changes in the second dose date, so please review your calendar before scheduling to ensure patient availability for the second dose appointment.

Healow Online Scheduling Instructions

 

If you are unable to use the online scheduling feature or prefer to call the office, you may do so at 309-662-0504 and select option 3.

 

We urge you to be patient when using the online scheduler or when calling the office after clinic dates are announced. Our call volume is extremely high at these times and we do our best to answer as quickly as possible.

 

  • Clinic dates will be available 1 week at a time.
  • Clinic dates will be posted on our Facebook page and here on our website.
  • All vaccine clinics are dependent on vaccine availability from the McLean County Health Department.

 


REQUIRED PAPERWORK


If you have a Covid-19 vaccine appointment scheduled, please see below for important information and required paperwork BEFORE your visit. 

 

In preparation for your/your child's upcoming Covid Vaccination, please download, print and complete the attached Pre-Vaccination Registration/Checklist and review the Vaccine Fact Sheet. Completion of the checklist and review of the fact sheet is required before you may receive your vaccination. If you are unable to print the checklist and/or review the fact sheet, please let us know. If time allows, we will mail the sheets to you or you may pick up at the office. Bring the completed checklist with you to your appointment. If you have questions or concerns about the checklist or fact sheet, please call the office at 309-662-0504 and select option 1 for the nurse.

UNACCOMPANIED MINOR FORM

Unaccompanied Minor Form
If your child is aged 16-17 and will be coming to the appointment on their own, we will need to have a completed Consent to Treat an Unaccompanied Minor form on file. If you have not previously completed this form for your child, please click below to complete the online form. This must be signed by a parent/legal guardian.

 


APPOINTMENT DAY CHECK-IN PROCESS

Please follow our current check-in process for NON-CLINIC traditional in-office appointments.

 

For in-Office Clinic appointments, patients should park in the South (large) parking lot and immediately enter the front of the building. We will have a check in desk for the vaccine clinic. After the patient is checked in, they will be guided to an exam room where a nurse will administer the vaccine. After your vaccination, you will be given your CDC Covid-19 Vaccination Record Card and will be given a timer while you wait in observation where a nurse will be monitoring for any post-vaccine reaction. When your timer is up, the observation nurse will assess the patient and release you! 

 

For Drive Through Clinic appointments, vehicles should enter through the North parking lot and join the line as it moves under the overhang at the front of our building. Please make sure everyone in the vehicle is wearing a mask. A staff member will check you in and collect your required paperwork. Then a nurse will administer the vaccine. It is helpful for the patient to be wearing a short-sleeve shirt and to have their arm exposed and ready when the nurse approaches. After your vaccination, you will be given your CDC Covid-19 Vaccination Record Card and will be given a timer and will exit through the South door to return to their vehicle in the Observation space where another nurse will be monitoring for any post-vaccine reaction. Any patients with concerns or needing help from the nurse should HONK and the observation nurse will come to you. When your timer is up, FLASH your headlights and the observation nurse will access the patient and release you! 

 

We appreciate your cooperation with our nursing staff to administer the vaccine to our patients in the safest manner for all.

  • Parents need to be prepared to assist in holding their child.
  • Nurses have the authority to decline to administer the vaccine if your child is uncooperative.

 

The above process will be the SAME for your 2nd dose appointment. However, you will need to bring the CDC Covid-19 Vaccination Record Card with you so we may document the 2nd dose. We will NOT be able to provide a second card, so please make sure you have it, and your Checklist for that appointment date with you when you come.
